OVERVIEW OF SERVICES

Care Management Services
Growing older is another part of our life's process. These years can be very satisfying, but they may also bring many challenges and problems. Stress, illness, loneliness and loss are but a few of these.

With so many people living longer and many families both near and far seeking support, geriatric care management provides advice and assistance to families in planning care for aging family members. Families turn to care management to define the needs of the elderly and find support services both in the home and in the community. JF&CS provides care management in part through participation in New Jersey EASE programs. Additional funding from the Jewish Federation and foundations help support many of our services for seniors.

Caring Connections is a care management service of JF&CS. It is most useful for families who live far away and are seeking hands-on assistance for elderly family members within the JF&CS service area. A JF&CS care manager will develop a care plan with the family and oversee the provision of services on a continuing basis, addressing changes in needs as they arise.

Care management services has several components:

  • Assessment: Assessment, education and consultation may be conducted with an adult child alone, or as a comprehensive evaluation with the older adult in his home, often with an adult child present. The needs of the older adult are considered along with the concerns of the family.

  • Plan of Care: The care manager will develop a plan of care, which identifies the needs of the old adult and what services are needed. This may include a home health attendant to assist with specific needs, social or medical day programs in the community, transportation, meals or other services.

    • Recommendations: As a result of the identified needs of the individual and the plan of care, the care manager will discuss recommendations with the older adult and the family. Information and education about applicable entitlement services, financial, and legal issues may be discussed as part of the recommendations. Referrals and resources are provided. Needs and care plans are reviewed regularly.

    • Counseling: The loss of loved ones, of productive lives, of good health along with diminishing independence often lead to depression, a common disorder among older adults. JF&CS Senior Adult Staff provide supportive counseling and treatment to seniors, both in our offices or in the home for those who cannot get out. Adult children often seek counseling services to balance the conflicting demands of both older and younger generations.

Group Services
It is often helpful to discuss with others specific challenges that are shared by others. Participants benefit from knowing they are not alone. Group members help each other in coping and moving forward. JF&CS offers a variety of groups, including:

  • Caregivers Support Group: Adult children and relatives who care for aging persons have many concerns. The group helps them learn how to care for themselves and their families as they face the demands of caregiving.
  • Bereavement Groups for Widows and Widowers: JF&CS addresses the needs of senior adults in their own groups. Seniors are happy to know that other participants share similar life concerns as they deal with loss.
  • How to be a Successful Mother-in-law: A support group to learn how to get along with your adult children and their spouses. These relationships are often challenging.

Information and Referral Services: JF&CS provides helpful and important information over the phone. Call for any of our services, speak with a social worker. Referrals for private and agency home health and homemaker services, information about housing, assisted living and nursing home facilities. Need a geriatrician? An attorney? Looking for financial services and entitlements? We can provide helpful information.

Kosher Meals on Wheels: Low cost hot lunch meals are available for seniors living in Fair Lawn and Elmwood Park, supported through Bergen County Division of Senior Services and Jewish Federation of North Jersey.

Transportation: JF&CS operates a 12 passenger van, provided by the state of New Jersey, to transport senior adults to a Federation sponsored kosher nutrition site, to grocery and mall shopping, and to agency functions and community events.
